How to become a jWIN business partner

We always welcomes new business partners. In order to be considered as a Business Partner, it is necessary tofill out and return to us the following forms:

Credit Application Form

Return Authorization Policy Agreement Form

Bank Release Form

Resale Certificate Form

All required forms, which are in Adobe’s PDF format, can be downloaded by clicking on the links below. Follow these instructions, and the statement below, as well.

1. Download the application forms.

2. Complete and sign the Credit Application and Resale Certificate.

3. Sign and return the original application forms by fax or standard mail.
